Cosmos Divine Care focuses on building independence, improving quality of life, and supporting participants to live safely and confidently in their community. All services are delivered in line with the NDIS Practice Standards and the NDIS Code of Conduct. Services Provided by Cosmos Divine Care Cosmos Divine Care provides support across the following NDIS service areas: Personalised Support and Assistance Support tailored to each participant’s needs, routines and goals, delivered with dignity and respect. Daily Personal Activities Assistance with personal care such as showering, dressing, grooming, mobility and daily routines to support independence at home and in the community. Participation in Community, Social and Civic Activities Support to engage in social, recreational and community activities, helping participants build connections, confidence and social skills. Assistance with Coordinating or Managing Life Stages, Transitions and Supports Support to navigate changes such as moving home, starting new services, or transitioning between life stages, including coordination with other providers. Assistance with Household Tasks Help with everyday household activities such as cleaning, laundry, meal preparation and maintaining a safe living environment. Assistance with Travel and Transport Support to attend appointments, community activities, education, employment and social outings safely and reliably. Assist Access and Maintain Employment Support to prepare for work, develop workplace skills, attend interviews, and maintain employment where appropriate. Development of Life Skills Support to build daily living skills, communication, decision making, budgeting, cooking, personal care, and social skills to promote independence.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

To be eligible for the NDIS, individuals must be under the age of 65, be an Australian citizen or hold a permanent visa, and have a permanent and significant disability that affects their ability to participate in everyday activities.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
