Cosmos Divine Care works closely with participants, families, carers and other professionals to ensure supports are tailored to individual goals, preferences and cultural needs. Services Provided by Cosmos Divine Care Cosmos Divine Care provides support across the following NDIS service areas: Personalised Support and Assistance Support tailored to each participant’s needs, routines and goals, delivered with dignity and respect. Daily Personal Activities Assistance with personal care such as showering, dressing, grooming, mobility and daily routines to support independence at home and in the community. Participation in Community, Social and Civic Activities Support to engage in social, recreational and community activities, helping participants build connections, confidence and social skills. Assistance with Coordinating or Managing Life Stages, Transitions and Supports Support to navigate changes such as moving home, starting new services, or transitioning between life stages, including coordination with other providers. Assistance with Household Tasks Help with everyday household activities such as cleaning, laundry, meal preparation and maintaining a safe living environment. Assistance with Travel and Transport Support to attend appointments, community activities, education, employment and social outings safely and reliably. Assist Access and Maintain Employment Support to prepare for work, develop workplace skills, attend interviews, and maintain employment where appropriate. Development of Life Skills Support to build daily living skills, communication, decision making, budgeting, cooking, personal care, and social skills to promote independence.
