Perth Integrated Health is one of the oldest most established private practices that originated from West Perth some 40 years ago to expanding its clinical medicine and then sports medicine outreach North of the River with our Northern Districts Centre in Balcatta, South of the River with our Southern Districts Centre in Applecross and to the East and the Perth Hills with our Gateway Hub in Midland. As our corporate name implies the word Perth Integrated Health refers to our focus in the quality provision of services within the Perth metro region and also through our Mobile Teams to the regions. We are a multi-national, multi-linguistic integrated team with people hailing from all sectors of the globe and have within our organization those who speak Mandarin, Malay, Indonesian, Dutch, Czech with English no doubt being the primary language. We are also closely linked and understand the traditions and needs of our First Nations brothers and sisters with many of us working with them in the health care and community space. Our Mobile team also covers area within the primary definitions of the Perth Metro region from Joondalup to the North, East covering all of the Perth Hills, West to the Indian Ocean coastline and South to Rockingham

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS can be difficult to navigate, it's important that you prepare and seek out assistance during your application process.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
