Hi, My name is Melvin Jap, I'm an Independent Support Coordinator based in Darwin. I have been working as a local support coordinator around Darwin for 2 years. Some of you might be familiar with me or maybe have even worked with me before. I really love what I do and am passionate about making a positive impact! :)  I have decided to take the next step and become an Independent Support Coordinator. I have attended Service Provider Awareness Training (SPAT) from Touching Base, and I'm able to assist with any sexual-related inquiry that you might have. Please let me know if you have goals around this! I'll be very happy to help. NDIS Support Category: Level 2 Support Coordination Please let me know if you don't have any support coordination funding or if it has been exhausted, I might still be able to help you. My special interests are Neurological Conditions, Wernicke Korsakoff Syndrome, Parkinson's Disease, Neurodevelopmental (Autism and ADHD), Acquired Brain Injury / Stroke, Genetic Disorders, Chronic Autoimmune Disorder and other Chronic Conditions. I’m also able to assist with Housing applications, Disability Support Pension (DSP), Companion Card, Disability Parking Permit, Guardianship and Public Trustee matters. I have extensive knowledge of mainstream services around Darwin. I like to use a person-centred approach and work with the other key stakeholders to identify any barriers that might hinder your participation and remove them, no matter if they are social, environmental, physical, systemic or even psychological. empowerment is an essential part of my practice. I also have a strong sense of advocacy when it comes to inequality and will provide my best expertise to make sure that you have the best outcome out of your NDIS plan. The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
