About this provider

Links2Community has been established with the goal of addressing the complexities of the National Disability Insurance Scheme (NDIS), Australia’s biggest social reform that has transformed the support system for people with disabilities. Our vision is to realize an all-inclusive, flexible and accessible community, where no one is socially isolated, irrespective of age, disability, language or cultural barriers.

We provide supports to NDIS participants in complex situations – eg: children in out of home care, people with disabilities involved in the criminal justice and juvenile justice system, people who are homeless or at risk of homelessness etc. We have a multicultural team who speak Arabic, Hindi, Punjabi, Bangla, Bengali, Urdu, Tagalog, Creole and Yoruba.

We have immediate capacity to provide Positive Behaviour Support, Psychosocial Recovery Coaching, OT Assessments, Social Work, Specialist Support Coordination and home modifications. We also have capacity for Social and Community Participation activities / Increased Social and Community Participation – our Support Workers have the skills and expertise in implementing therapy and capacity building programs developed by Allied Health Practitioners.
