The Specialist Support Coordination team based in Brisbane focuses on addressing barriers and reducing complexity in the support environment while assisting our NDIS participants in connecting with support and building capacity and resilience in their communities. This approach supports a service delivery model where service delivery is planned, integrated, goal-oriented, and accountable while ensuring each participant receives culturally appropriate services in an effective and efficient manner that is consistent throughout the NDIS system. We take a holistic and trauma informed approach to delivering two levels of Support Coordination under the NDIS: Support Coordination: ongoing support throughout the duration of your plan, for people with more complex needs who may require additional support to connect with providers. Specialist Support Coordination: higher intensity Support Coordination. Our multidisciplinary team partners with you and your support network to action your NDIS plan and reach your potential.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

To be eligible for the NDIS, individuals must be under the age of 65, be an Australian citizen or hold a permanent visa, and have a permanent and significant disability that affects their ability to participate in everyday activities.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS can be difficult to navigate, it's important that you prepare and seek out assistance during your application process.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
