About this provider

A support coordinator assists NDIS participants to understand and use the supports in their NDIS plan by:

- supporting participants in pre-planning meetings (including their first planning meeting)

- helping participants and their plan nominee to understand their NDIS plan

- educate participants and plan nominees to use the NDIS portal

- identify service providers to work with the participants NDIS plan, goals and within the participants allocated budget

- support participants to aim to live as independently as possible within their community using a collaborative approach to care needs

- write reports required for use with the NDIS and gather relevant information to send to NDIS

- attend and advocate at all planning meetings for future plans and relevant funding using an evidence base model

Allied health practionners support participants in the following areas -

- Dietitics

- Occupational Therapy (Functional Care Assessments only)

- Physiotherapy

- Counselling

- Speech Therapy

- Allied Health Assistant

- Exercise Physiology
