We are located in Caroline Springs, a Western Suburb of Victoria and happy to work anywhere we can( Aged care services, Daycare centres. and so on). Here is a brief summary of the services which we offer. *Household task /personal care *Assistance with travel and transport(assistance getting to a medical appointment, attending events and activity participation *Community Nursing care. Therapeutic support ( we have the service of Physio, OT, and Exercise physiologist) Participate in Community (To come out and about in the community, informal support with family and friends) Group/Centre Activities Innov Community Participation Daily Tasks /Shared Living Assist personal activities Assist-life stage.Transition We also have staff that speak other languages, such as Arabic, Bosnia, and Croatia. Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
