Goldilocks Community Health is an established registered NDIS provider since 2020 offering the following quality support service delivery models of care: - support coordination Levels 1, 2 and 3 - psychosocial recovery coaching - mental health interventions and collaborations - home and living pathway creation SDA assessments - needs assessment and plan implementation of funded services - Self-directed STA/Respite options created in collaboration with participants who can use their own support teams - SLES and Specialised Employment support - CB improved daily living for participants and their families (ask about our budgeting programs and dress for success just to name a few - CB Improved skill development for participants, and carers - to better understand NDIS guidelines, plan utilisation, evidence based practice and goal achievement - local network of providers to include therapists, support workers, mainstream services, and others We are committed to being the "just right" service and look forward to discussing your individual needs We believe in quality over quantity service outcomes every step of the journey and embrace the opportunities to work at the participants pace in their space.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS can be difficult to navigate, it's important that you prepare and seek out assistance during your application process.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
