We are passionate about giving the power back to individuals with a disability to realise their potential and achieve their goals. The purpose of Coordination and Care Services is that we are able to create an environment that builds resilience and confidence within our participants so they can achieve their optimum level of independence. We believe Support Coordination isn’t about hand-holding, case management or tea and sympathy. At Coordination and Care Services, we believe that every single action we deliver in Support Coordination honours the social model of disability, the independence of the participant and the choice and control that supports the NDIS. *OUR VALUES* Community • We are accountable to make and build stronger relationships within our community. We recently changed to Coordination and Care Services Pty Ltd – Support Coordination and have Jacinta for our boys. She has been fantastic without her help we would be just putting up with a plan that doesn’t work and my son would not be getting the services he needs. She helped us get a new review. Nothing is too much trouble and her knowledge of the whole NDIS system is second to none. She looks at the whole picture and how it affects the whole family. You are not just a client she develops a connection with you.

Finally, a Support Coordinator that actually coordinates support. Dila is a breath of fresh air and a pleasure to work with. Will take the time to answer my many questions, returns my calls if she is busy, investigates my inquiries and will always try to find ''can do'' ways to make things happen for me. Highly recommended.

My new support coordinator is really good. Dila takes the time to explain the things to me that I find hard to understand. She will listen to me and make sure that all my goals are met. Dila makes sure that I get the best out of my NDIS funding to make my life better. Sometimes she just rings up to say hi and see how I'm going which is really nice.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
