Coordination and Care Services is a registered NDIS provider supporting those with a disability to enhance their potential in order to live fulfilling and independent lives. We believe in a person-centred support system that ensures that our staff treat our participants the way they would like to be treated, giving participants the freedom to make their own delivery choices and access their preferred allied health services. We are passionate about giving the power back to individuals with a disability to realise their potential and achieve their goals. The purpose of Coordination and Care Services is that we are able to create an environment that builds resilience and confidence within our participants so they can achieve their optimum level of independence. We believe Support Coordination isn’t about hand-holding, case management or tea and sympathy.
