360 Health + Community is a community health service provider that runs a variety of programs that focus holistically on the physical health, mental health and social wellbeing of our clients and the wider community. Our Allied Health programs are carried out by a team of highly skilled and knowledgeable clinicians who help our clients to work towards chronic health prevention, condition and pain management, weight loss, healthy eating and diet optimisation, nutrition and exercise. Our allied health services include: - Diabetes Education - Dietetics - Exercise Physiology - Occupational Therapy - Speech Pathology - Pain Management - A Coordinated Endocrinology and Diabetes (CEDS) program - A Healthy Lifestyle Supports initiative 360 Health’s highly qualified team of Mental Health clinicians provide professional support to address a wide range of mental health issues such as stress, anxiety, depression and other mental health issues. In collaboration with our clients, their family, friends and other health professionals, our clinicians develop and work towards recovery goals that can improve the clients’ wellbeing and quality of life. The mental health support services are available for individuals and families include: • Commonwealth Psychosocial Support (CPS) • ALIVE Suicide Prevention, and • Headspace Mandurah, Rockingham and Fremantle. As a registered NDIS provider, 360 Health teams support and work with people living with disability and specifically, psychosocial disabilities to create practical and sustainable strategies for the home, school and community environment to ensure that their physical health, mental health and social wellbeing needs are met, and they are able to live life to the fullest. Our NDIS services include: • Therapy Services in collaboration with our Allied Health clinicians • Recovery Coaching, and • Support Coordination.

The NDIS provides funding to eligible people with disability to gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

To be eligible for the NDIS, individuals must be under the age of 65, be an Australian citizen or hold a permanent visa, and have a permanent and significant disability that affects their ability to participate in everyday activities.

Your NDIS plan revolves around your personal life goals and aspirations. It is crafted based on the information you share during your planning session with your Local Area Coordinator (LAC). Within your plan, you will find a breakdown of the various supports required to help you reach your goals, along with the specific funding allocated to each support category.

Your NDIS plan is tailored to your needs and is reviewed regularly to ensure it continues to meet your changing circumstances and goals.

The NDIS requires service providers to register with them to allow for direct invoicing claims for NDIA-managed clients. However, not all providers opt for registration, meaning there could be a suitable unregistered provider out there that can offer you supports. Access to these unregistered providers is only available if you self-manage your funds or have a Plan Manager.
